What Is the 50058?

Every household receiving Section 8 assistance has a HUD Form 50058 on file. Your PHA completes this form at initial enrollment and updates it at every annual recertification and interim change. It's the master record that drives everything about your benefits.

Think of it as your Section 8 blueprint. If a number on the 50058 is wrong, your rent is wrong.

What's on the Form

Household composition. Every person living in the unit — names, dates of birth, Social Security numbers, relationships, disability status, student status, and citizenship/immigration status.

Income. All counted income for every household member: wages, Social Security, SSI, pensions, child support, and other sources.

Assets. Bank accounts, real property, retirement accounts, and other assets. If total assets exceed $5,000, imputed income from assets may apply.

Deductions. Dependent deductions ($480 each), elderly/disabled deduction ($400), childcare expenses, disability assistance expenses, and medical expenses (elderly/disabled families only).

Rent calculation. The math that produces your Total Tenant Payment: annual income minus deductions, divided by 12, multiplied by 30% (or 10% of gross monthly income, whichever is greater), with the minimum rent as a floor.

Housing information. Unit address, number of bedrooms, contract rent, utility allowance, and HAP (Housing Assistance Payment) to the landlord.

Common Errors to Watch For

Wrong household members. Someone who moved out is still listed (increasing your income), or a new dependent isn't listed (missing the deduction).

Incorrect income. Old income data, wrong amounts, or income that should be excluded (like student financial aid or foster care payments).

Missing deductions. The dependent deduction, elderly/disabled deduction, or medical/childcare expenses weren't applied.

Wrong utility allowance. The utility allowance doesn't match your actual utility responsibilities. If you pay for heating but the form says the landlord does, your allowance is too low.

How to Get and Review Your 50058

You have the right to request a copy of your 50058 from your PHA. Ask for it at your next recertification, or call and request one. Some PHAs provide it automatically; others require a written request.

When you get it, go through each section line by line. Check: Are all household members correct? Is the income accurate and current? Are all deductions applied? Does the utility allowance match your actual utilities? Does the final rent calculation match what you're being charged?

What to Do If You Find an Error

Notify your PHA caseworker in writing immediately. Describe the specific error and provide documentation supporting the correction. If the error resulted in you overpaying rent, the PHA should recalculate and provide a credit or refund for the overpayment period.

If the PHA disagrees or doesn't act, you have the right to request a hearing.
